Obtenga ayuda en persona para revisar sus opciones de cobertura de salud y elegir el plan que mejor se adapte a sus necesidades. Para facilitarle este proceso, Access Health CT ofrece sesiones gratuitas, individuales con profesionales capacitados que pueden orientarle sobre sus opciones, responder a sus preguntas y ayudarle a inscribirse en el plan adecuado para usted y su familia.

Use esta página para programar su cita en el horario que más le convenga. Ya sea que se esté inscribiendo por primera vez, renovando su cobertura o explorando nuevas opciones de planes, nuestro equipo está aquí para brindarle apoyo personalizado en cada paso del proceso.

Lo que necesita para completar una solicitud

Por favor, traiga la siguiente información para usted y para todas las personas de su hogar que estén solicitando cobertura:

  • Fechas de nacimiento
  • Números de seguro social
  • Visa, tarjeta de residencia permanente (Green Card) o documentos de inmigración
  • Formulario W-2 y/o 1040, 1099 más recientes
  • Comprobante de cobertura de seguro actual
  • Recibos de pago, declaraciones de pérdidas y ganancias u otra información laboral para confirmar los ingresos de todas las personas incluidas en su declaración de impuestos
